Show your Valor pride everywhere you go with Evangel University customized license plates!
Now launching EU customized license plates for alumni, students, employees, donor, and friends currently residing in Missouri!
The first step of the process is to gather a minimum of 200 applicants to show that there is interest in this unique custom plate.
How to get started!
- Complete the brief interest form below. Feel free to share the link with your family and friends so they can also proudly support EU!
- When we receive the required minimum of 200 interest forms, a member of our team will send you an Application for Missouri Personalized and Special License Plates form (Form 1716) that you will need to complete and return to EU via email, fax, or postal mail. The EU team will also send you a unique link where you can pay the $15 custom license plate fee. If you prefer to pay the $15 custom license plate fee via postal mail, you may send a check or money order to the address listed below.
- After completing Form 1716 and returning to EU, please make a minimum $25 tax-deductible donation to EU. This is a MO Department of Revenue requirement to receive a university-branded custom license plate. You may give online here or mail a check or money order to:

- EU will submit the forms and payment to MO DOR on your behalf and you will receive your EU customized license plate(s) in the mail!
Note that you DO NOT have to wait for your license plate to expire before ordering an EU customized license plate!
